It is exactly that. Not only is the cover plate missing, but the washer pump / actuator is just lying inside the bumper cover loosely... as in held in place by nothing other than the hose.
I called our dealership to tell them the issue, and instead of getting the ball rolling on part ordering, they are telling me to bring it to them Friday so that they can be sure they order the right parts. Seems like an inefficient use of time if this is a known issue.
I'm not surprised they didn't order the part. They have to confirm it themselves. Yeah, it's a pain, but I can see their side (if they order the incorrect part(s), then what?).

I'm sorry for the frustration. The Studio is doing their due diligence to make sure they have the correct part ordered, which they cannot do without an official inspection/diagnosis. Please keep us informed tomorrow after your appointment as to what they say and the status of the part needed.
Yes the mounting bracket inside the bumper is too weak in original design and needs to be upgraded. When it comes loose the painted cover rips off in the slipstream and needs to be replaced AND repainted. Removing the relay will disable the washers.
My car was built on July 21 as a 2018 model. I hope and pray the washers are already upgraded but I am not sure.
So last week I decided to pull the relay as the headlight washer cover is getting more and more loose.
I didn't realize that the engine compartment fuse box is not easily accessible without taking some stuff apart (there are hoses and cables obscuring most of it which can't be easily moved).
I got the cover off and located the relay thanks to MacGeek's photos above. I could just about squeeze my fingers into the box around the obstructions. With one finger of each hand I pried the relay loose.
Then disaster! It pops off, I lose grip and it tumbles down into the engine compartment. OMG, what if that's the wrong relay and important stuff now doesn't work?
I can see half of the relay way down inside the compartment. Certainly can't reach it without removing half of the engine. Tried barbecue tongues...too big to fit in the gap. Magnetic extension on a screwdriver....brass connector son no good. Macgyvering is all I have now. Wifey finds an old kids' magic wand. Put some duct tape sticky side out on the end. Reach down with it and it sticks to the relay.
Oh so gently pull it up through the engine components. Kind of like that game of Operation. Don't set off the buzzer!
I get it, put it away safely for when I get the dealership to replace the brackets.
Oh....and it works like a charm. The headlight washer is disabled for now. Whew!
Unfortunately my driver side washer cover has started doing this as well. The dealers are very familiar with this issue and are aware of what it takes to resolve it. Mine will be fixed shortly .....
Another thought about the washer operation. Why is it that the headlight washers operate every time you want to spritz the windshield? In my RS4 you held the washer stalk for two seconds or more to activate the headlight washer as well.

I?m not with my car now so can?t be precise, but on the left side (facing the engine compartment) near the front of the car. Black box under some hoses and cables. Be careful as per my post above....maybe use pliers instead if your fingers for a better grip.

I asked my dealer about it when I took delivery and he had no idea what I was talking about. They also haven't sold very many QVs, but I was surprised. We'll see what happens with mine but so far so good.
